Precision Care is an APRN-owned remote medical service. We contract with a client — a physician’s office, retail clinic, FQHC, or other healthcare organization and offer after-hours call services and inbox management with telephonic or video urgent visits as an extension to your practice.

How it works.

After-hours

1. The patient phones in.

A community dwelling patient phones the call center, who sends the information to the Precision provider after hours. The provider accesses and reviews the medical record remotely through a secure connection through your organization and then speaks with the caller. A communication note, or telephonic tele visit is documented. If the patient is going to the hospital, the backup provider from your organization is alerted and medical records are sent to the ED. Any controlled substance refills are referred to the organization’s backup provider.

2. If the issue hasn’t been resolved…

The patient is referred to the primary care team for follow up the next business day to address the patient’s concerns and the team is alerted about the issue the patient needs addressing. If the issue cannot be resolved without an in-person assessment, some organizations have RN’s on call or a Paramedical team to deploy and do a collaborative visit with the Precision providers bedside.

Inbox Management

1. A Precision Care provider accesses the inbox and identifies urgent messages/lab results that had been assigned to the primary provider. The Precision Care provider accesses and reviews the medical record remotely through a secure connection through your organization and then sets up a telephonic or video appointment to address the urgent need. A visit note documented and the contracted organization bills the insurance for the service.

2. The patient is referred to the primary care team if an in person visit is required or further follow up is needed.

Precision Care, LLC are contracted by your healthcare organization and do not bill the patients.

What we don’t  do.

Precision Care does not own an electronic medical record, or keep documentation outside the electronic medical record system owned by our clients.

Precision Care does not have an NPI, therefore it does not have patients or bill patients or insurances. It has a flat fee contract with the client organization who manages all of its own billing.
